In addition, 131 I therapy should be avoided or postponed in patients with active Graves ophthalmopathy, especially if they are cigarette smokers, since a recent prospective study10, 11 has demonstrated worsening in such patients treated with 131I compared with those treated with antithyroid drugs or subtotal thyroidectomy. The dose of 131I used to treat Graves hyperthyroidism ranges from 185 to 555 MBq 5-15 mCi ; , depending on the size of the goiter and the magnitude of uptake of an antecedent tracer dose of 131I. With toxic nodular goiter, larger doses are required to achieve a euthyroid state. Pretreatment with propylthiouracil, but perhaps not methimazole, may reduce the one-dose cure rate of 131I through a putative radioprotective effect.12, 13 Accordingly, methimazole should only be used before 131I treatment in patients with severe hyperthyroidism or a very large goiter to forestall exacerbation of hyperthyroidism as a result of the transient ra and singulair.
