Their actions. These differences are predictable from their chemical structure. The phenothiazines share a common tricyclic nucleus; on the ten position in the tricyclic nucleus, the attached radical may be either an aliphatic dimethylamino ; chain or a heterocyclic ring piperazine and piperidine ; .17 The clinical importance of these groups is that the dimethylamino phenothiazines promethazine and chlorpromazine ; are less potent and have significantly more sedative action than the piperazines prochloperazine and perphenazine the latter have correspondingly greater antiemetic properties. The phenothiazines are predominantly antidopaminergic agents with moderate antihistaminic and anticholinergic activity. Because of this widespread activity these agents are found to be effective and therefore popular for prophylaxis and treatment of emesis. Chlorpromazine possesses significant prophylactic antiemetic properties; however, in effective doses, undue sedation and hypotension are frequent side effects.18 Promethazine has been very popular as a premedicant; other than its antiemetic action it has the advantage of anticholinergic and potent antihistaminic properties which make it a useful premedicant for asthmatic patients. Promethazine is an effective prophylactic antiemetic but tends to cause greater delay in awakening from anaesthesia than the piperazines. 19 When promethazine is administered for treatment of active vomiting, an intravenous dose of 12.5 mg has been shown to be effective with little change in blood pressure, although with varying levels of subsequent sedation. 12 Chlorpromazine has no effect against motion sickness, and promethazine is the most potent of the phenothiazines for this problem. When motion sickness is an important etiological factor in postoperative emesis, this drug would be preferable to hyoscine because of its longer duration of action. Similarly, promethazine is the prophylactic antiemetic of choice for ear sugery. However, chlorpromazine and promethazine administered after anaesthesia have the capacity to induce sleep in patients who are still recovering from other anaesthetic agents. This can be potentially hazardous while vomiting is still not effectively controlled. Prochlorpeazine and perphenazine have justifiably been the most popular of the phenothiazines for both prophylaxis and treatment of vomiting associated with anaesthesia. A large study was conducted to determine which factors from a biological-psychological-social model predict HRQL after medical or invasive cardiovascular interventions, and which accounts most of the unexplained variance between treatment methods. Drugs such as dicyclomine Bentyl ; , prochlorperazine Compazine ; , promethazine Phenergan ; , methocarbamol Robaxin ; , and clonidine Catapres ; can ameliorate the unpleasant symptoms of withdrawal. No evidence supports the idea that noxious withdrawal during detoxification improves one's outcome. A debate surrounds whether to use active tapers when it is not necessary and whether to taper slowly or rapidly. Additionally, an active debate is taking place about how long to leave patients on drugs that are useful for tapering off addictive compounds. The detoxification of opiates is accomplished through a substituted taper of long-acting opiod agonists such as methadone. Alternative agents that can be used include buprenorphine, a mixed opiod agonist-antagonist and clonodine, an alpha-2 agonist, which can bring symptomatic relief of heroine withdrawal. A good deal of debate exists over how gently to taper these drugs, whether they need to be used only on an inpatient basis, how much clonodine to use, and whether or not opiate addicts all need methadone maintenance or if some can be drug-free after a taper.

Gallium-67 Imaging results may be distorted because of the rapid urinary excretion of deferoxamine-bound gallium-67. Discontinuation of deferoxamine 48 hours prior to scintigraphy is advisable. Information for Patients Patients experiencing dizziness or other nervous system disturbances, or impairment of vision or hearing, should refrain from driving or operating potentially hazardous machines see ADVERSE REACTIONS ; . Patients should be informed that occasionally their urine may show a reddish discoloration. Carcinogenesis, Mutagenesis, Impairment of Fertility Long-term carcinogenicity studies in animals have not been performed with deferoxamine. Cytotoxicity may occur, since deferoxamine has been shown to inhibit DNA synthesis in vitro. Pregnancy Category C Delayed ossification in mice and skeletal anomalies in rabbits were observed after deferoxamine was administered in daily doses up to 4.5 times the maximum daily human dose. No adverse effects were observed in similar studies in rats. There are no adequate and well-controlled studies in pregnant women. Deferoxamine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Nursing Mothers It is not known whether this drug is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when deferoxamine is administered to a nursing woman. Pediatric Use Pediatric patients receiving deferoxamine should be monitored for body weight and growth every 3 months. Safety and effectiveness in pediatric patients under the age of 3 years have not been established.
