PHARMACOL. 2003 55 11 ; - summ in ENGL As a part of our search for the active metabolite from the bark of Magnolia officinalis Magnoliaceae ; , the aqueous extract was orally administered to rats, and metabolites in the urine were analysed by a high-performance liquid chromatograph equipped with a photodiode array detector. When the extract was given to rats, five metabolites sinapic acid-4-O-sulfate 1 ; , sinapic acid-4-O glucuronide 2 ; , sinapic acid 3 ; , 3-[2 , 6-dihydroxy-5 2-propenyl ; [1, 1 -biphenyl]-3-yl]- E ; -2-p ropenoic acid 4 ; , and an unchanged form, magnolol 5 were detected in the urine. It was revealed that metabolites 1-3 and 4 were respectively derived from syringin and magnolol contained in the extract. In a human urine sample, metabolites 3-5 and dihydroxydihydromagnolol 6 ; were detected. These structures were identified by a combination of spectral methods and or by comparison with authentic compounds obtained by synthesis. Among these free form metabolites 3-6 ; , acute treatments with magnolol and dihydroxydihydromagnolol 50-100 mg kg-1 , i.p. ; attenuated the forced swim-induced experimental depression in mice. The results indicated that magnolol and dihydroxydihydromagnolol were the antidepressant constituents of Magnolia officinalis. 418. Injectable bisphosphonates in the treatment of postmenopausal osteoporosis - Sartori L., Adami S., Filipponi P. and Crepaldi G. [Dr. L. Sartori, Clinica Medica I, Dept. of Medical Surgical Sciences, University of Padova, Via Giustiniani 2, 35128 Padova, Italy] - AGING CLIN. EXP. RES. 2003 15 4 ; summ in ENGL Osteoporosis is a "silent" disease and the patient has usually no clue of it until the occurrence of a fragility fracture. Prevention requires a continuous daily treatment that could be uncomfortable to the patient. Besides the recently introduced weekly oral schedules, injectable bisphosphonates have often been used as an off-label option to ameliorate compliance. In general, although with different efficiency, almost all injectable bisphosphonates can improve bone mineral density and suppress bone resorption markers. The effect of intravenous infusions of bisphosphonates are, to a large extent, similar to equivalent intramuscular administrations, but doses and dosing intervals represent the critical issues. Pain at the injection site and acute phase reactions are relatively common to intramuscular clodronate and intravenous infusions of nitrogen-containing bisphosphonates, respectively. Under certain circumstances, intermittent treatment with injectable bisphosphonates might represent a feasible alternative when compliance is at risk. 2003, Editrice Kurtis. 419. Clerocidin alkylates DNA through its epoxide function: Evidence for a fine tuned mechanism of action - Richter S., Gatto B., Fabris D. et al. [M. Palumbo, Dept. of Pharmaceutical Sciences, University of Padova, via Marzolo 5, 35131 Padova, Italy] - NUCLEIC ACIDS RES. 2003 31 17 ; - summ in ENGL Clerocidin CL ; is an effective topoisomerase II-poison, which has been shown to produce DNA depurination and strand breaks per se at the guanine G ; level. To elucidate the roles played by the different functional groups of CL in the reactivity towards nucleic acids, we investigated CL derivatives with key structural modifications. The derivatives were reacted with plasmid, single double-stranded DNA and isolated 2 -deoxy-guanosines dG ; . We show here that an intact oxirane ring is essential to achieve DNA modification and depurination. Through HPLC MS and MS MS techniques we were able to unambiguously characterize adducts obtained by reacting isolated dG and single- double-stranded DNA with the drugs, indicating beyond reasonable doubt that the structure of a typical adduct is formed by epoxide alkylation at N7 of with subsequent loss of the pentose unit. Further, we showed that reduction of vicinal carbonyl functions affect drug activity to a large extent. Our findings demonstrate that the characteristic DNA-alkylating properties of CL arise from mutual action of the functional groups present in this molecule. Its oxidation state seems crucial to modulate the rates of reactivity by finely tuning the strain applied on the oxirane ring. Section 30 vol 126.2, for instance, omnicef infant.

Have yourself tested through Integrative Reflex Testing to find out how much Candida is present in your body and what areas are affected by it. Many clients notice a great improvement in as much as a few days and many within three weeks. Still it takes an average of six to twelve months to eradicate a candida overgrowth. The candida probably became well established before it was identified which is why treatment must be persistent. The major symptoms to improve are headaches, diarrhea, emotional and behavior problems, vaginitis and chemical sensitivities. When fighting a Candida infection a person must be systematic and disciplined because the miseries of the problem do not cease immediately with treatment. Candida "die-off" can cause some very uncomfortable symptoms temporarily. These symptoms disappear soon after beginning the diet and

P171 HEIDELBERG RETINA TOMOGRAPH IN NORMAL SUBJECTS AND PATIENTS WITH PRIMARY OPEN ANGLE GLAUCOMA AND OCULAR HYPERTENSION Marieta Konareva-Kostianeva, M. Atanassov Department of Ophthalmology, Medical University, Plovdiv, Bulgaria PURPOSES To compare optic disc and retinal nerve layer RNFL ; in normal, ocular hypertensive OH ; and glaucomatous eyes POAG ; undergoing Heidelberg Retina Tomograph - HRT and to determine the influence of optic disc size on the other topographic parameters. PATIENTS AND METHODS One hundred seventy-three eyes of 99 consecutive normal n 34, mean age 58 8 years, 61 eyes ; , glaucomatous n 52, age 61 10 years, 88 eyes ; and ocular hypertensive subjects n 13, age 61 17 years, 24 eyes ; were enrolled. Each patient underwent complete ophthalmic examination, achromatic automated perimetry and Heidelberg Retina Tomograph. Differences in the mean values were evaluated by analysis of variance, with multiple comparisons by the Bonferroni's method. RESULTS Glaucomatous eyes differ significantly p 0.001 ; from healthy and ocular hypertensive eyes regarding the following topographic HRT parameters: cup area, rim area, cup volume, rim volume, CD ratio, cup shape measure, RNFL thickness and cross-sectional area. Mean measurements of topographic optic nerve parameters of ocular hypertensive eyes showed no differences from those of normal eyes. No differences were found in disc area among the three groups 2.19 0.55 vs. 2.20 0.49 vs. 2.16 0.40 mm2, respectively ; . Disc area correlates significantly p 0.05 ; and positively with rim area and rim volume in all studied groups. CONCLUSIONS Our results suggest that POAG is distinguishable from controls and OH based on evaluation of HRT topographic parameters. Optic disc and RNFL appearance shows no difference between normal and ocular hypertensive eyes and

Introduction In all anxiety disorders, symptoms are present which suggest the involvement of the autonomous nervous system ANS ; . In panic attacks, which can occur in several anxiety disorders, ANS symptoms are prevalent, such as palpitations, chest pain and shortness of breath. In panic disorder PD ; , which is characterized by spontaneous panic attacks American Psychiatric Association 1994 ; , several studies have investigated the involvement of the ANS Roth et al 1986; Weissman et al 1987; Yeragani et al 1989; Katerndahl 1990; Papp et al 1993; Seier et al 1997; Bystritsky et al 2000 ; . Recent studies have used the analysis of heart rate variability HRV ; as a tool to study the functioning of the ANS. This analysis of a simple, noninvasive electrocardiogram ECG ; elucidates the regulatory processes that maintain cardiovascularsystem stability Appel et al 1989; van Ravenswaaij-Arts et al 1993 ; . HRV is primarily controlled by the continuous interplay of the sympathetic and parasympathetic vagal ; branches of the ANS Friedman and Thayer 1998b ; . Spectral analysis of HRV quantifies the relative influence of each branch on heart rate HR ; , as these rhythms occur at different frequencies. Two frequency bands are usually distinguished: the high frequency band HF: around 0.25 Hz ; is thought to be mediated by cardiac vagal tone, which depends on respiration, and a low frequency band LF: around 0.10 Hz ; which is mediated by both branches of the ANS Akselrod et al 1981; Pomeranz et al 1985 ; . The ratio of LF power to HF power LF HF ; is commonly seen as an index of cardiac sympathovagal balance Pagani et al 1986 ; . In several studies it has been reported that PD patients are characterized by a reduced HRV, reduced HF power, increased LF or an elevated LF HF ratio; for an extensive review see Friedman and Thayer 1998b ; . These HRV abnormalities seem to point to a disturbed balance between the vagal and the sympathetic branches of the ANS, with possible vagal hypoactivity and sympathetic overactivity. Not all studies have consistently shown this pattern: one research group reported no HRV abnormalities in PD patients Stein and Asmundson 1994; Asmundson and Stein 1994 ; . Possible explanations mentioned for these discrepant findings were factors such as apprehensiveness of subjects at the time of testing Ito et al 1999 ; , age, fitness, illness duration and illness severity Stein and Asmundson 1994; Asmundson and Stein 1994 ; , or even methodological differences in the analysis of HRV Friedman and Thayer 1998b ; . HRV has not been investigated very thoroughly in other anxiety disorders. To date, no studies in Obsessive Compulsive Disorder OCD ; have been carried out, using spectral analysis of HRV. Patients with OCD are characterized by intrusive thoughts and irresistible urges to perform ritualized actions. When patients attempt to control these thoughts and urges, anxiety is often reported and elevated ANS activity is evident Boulougouris et al 1977; Rabavilas et al 1977; Zahn et al 1996 ; . Earlier studies on ANS functioning in OCD patients have shown conflicting results: some studies found higher levels of skin conductance or heart rate, indicative of elevated autonomic arousal Insel et al 1985; Benkelfat et al 1991 ; , whereas others found no differences with healthy controls Hollander et al 1991 and

Seizures 1.2% ; resulted in death, an incidence of 2.68 per 100, 000 population; other studies have suggested a relatively high risk of death in persons with epilepsy.2 In 1885, Gowers3 wrote, "spontaneous cessation of seizures is an event too rare to be anticipated in any case . each attack facilitates the occurrence of another." This statement implies that seizures lead to other seizures, and thus treatment is necessary. However, little clinical evidence suggests that a single seizure causes brain injury except for status epilepticus. Some reports suggest that seizures may permanently or temporarily impair cognition, but there is no consensus that treatment with AEDs will alter this impairment. An important issue associated with seizures is the psychosocial stigma. Often, the consequences of seizures are age-related. Infants and toddlers may be more likely to be affected by consequences related to the type of seizure eg, head injury from an atonic seizure ; . Children may feel the effects of overprotection resulting from the family's fear of recurrence or inability to get day care or babysitting. Adolescents can be greatly affected by recurrent seizures. Common concerns include social isolation, driving limitations, and school and sports restrictions. Adults are concerned primarily with driving limitations, fewer employment opportunities, difficulty obtaining medical and life insurance, limited social opportunities, and the inability to meet their responsibility as providers. Thus, these psychosocial consequences must be factored into the decision about why seizures should be prevented. RISKS OF TREATMENT Countering the arguments for preventing seizures is the fact that AEDs present some risks to patients taking them. The risk of idiosyncratic serious complications is low; fatal, for example, dosage of omnicef.
