
With the passage of the Insider Trading Sanctions Act of 1984, the SEC obtained the ability to seek civil money penalties for insider trading. Based on the perception that the securities industry was not exercising sufficient vigilance in detecting and preventing insider trading, in 1988 Congress enacted the Insider Trading and Securities Fraud Enforcement Act "ITSFEA" ; . Pursuant to ITSFEA, broker-dealers, investment advisors, and other securities firms must establish policies and procedures designed to detect and prevent insider trading by their employees. Congress also gave the SEC the authority under ITSFEA to request that a court impose substantial financial penalties not only on a person who engaged in insider trading, but on the firm that employed the insider trader and his supervisors if they knew or recklessly disregarded the fact that he was likely to engage in insider trading and failed to take sufficient steps to prevent it. ITSFEA also authorizes the SEC to seek penalties against the firm and supervisors if they intentionally or recklessly failed to establish sufficient policies and procedures required to prevent insider trading and that failure led to the employee's insider trading. Last June, the SEC obtained a $10 million penalty from Morgan Stanley & Company Inc. for an alleged failure to maintain and enforce sufficient written policies and procedures designed to prevent the misuse of material, nonpublic information by Morgan Stanley and its employees. In March 007, Banc of America Securities LLC agreed to pay a $6 million penalty in settlement of alleged violations of the same securities-firm compliance requirement. AP ; -- Promoted as a great way to dispose of treated waste, the sewage sludge sold to homeowners to spray on their lawns and gardens may also be adding drugs, flame retardants and other chemicals to the landscape, according to a study. Chad Kinney, an assistant professor of chemistry and biochemistry at Eastern Washington University, found dozens of medicinal, industrial and household compounds in treated sewage sludge, also known as biosolids, that government agencies sell as lawn-and-garden enhancements. ''No matter what biosolid we looked at, there were some of these compounds in it, '' said Kinney, whose research on the subject was published in online editions of the journal Environmental Science & Technology. The U.S. Geological Survey's Toxic Substance Hydrology Program supported his work, which began while he was a postdoctoral fellow at the U.S. Geological Survey. Kinney and his team studied nine biosolid products from seven states: Washington, Arizona, Wisconsin, Kansas, Colorado, Texas and Iowa. The scientists found that it didn't matter what wastewater treatment method was used, 25 compounds were found in each of the samples. They were looking for 87 different compounds and found 55 in one or more of the biosolids and at least 30 in each of the samples. The product with the most compounds had 45. Although government regulators and health officials said there is no immediate risk to public health, the study's authors called for more research on the long-term impact on the environment. ''We've been using biosolids for over 30 years safety, '' said Peggy Leonard, biosolids program manager for King County's waste treatment division, which produces GroCo. ''As far as I know, there is no risk.'' Thomas Burke, a professor of public health policy at Johns Hopkins University in Baltimore, said Kinney's research and other studies should be a wake up call for the U.S. Environmental Protection Agency. The antiprotozoal drug pentamidine is second-line therapy for Pneumocystis carinii pneumonia in patients with AIDS, in whom this opportunistic infection is one of the most important causes of morbidity and mortality. The drug is excreted unchanged in the urine, and can be nephrotoxic, partly because of severe hypotensive reactions. It also causes hypoglycaemia by stimulating insulin release. About 30% of patients with AIDS given infusions of the drug become hypoglycaemic during therapy. The hypoglycaemia sometimes recurred or persisted for days after the drug was stopped. It is usually symptomatic, and can prove fatal. High drug dosages and significantly impaired renal function are important precipitants. Inhaled pentamidine can also cause hypoglycaemia. Because the drug damages the pancreatic B-cells, hyperglycaemia can follow pentamidine treatment, usually after several weeks or month, and sometimes in patients who have suffered from episodes of hypoglycaemia previously. Pentamidine is a potentially toxic drug and it should only be administered by those experienced in its use.

The main advantage of voriconazole is that it can be given orally and has a higher bioavailability than itraconazole. Inclusion criteria for the trials with patients with invasive aspergillosis were definite or probable IA. Is the data for use in salvage therapy sufficient enough to define the role of voriconazole as second-line post-treatment failure? The duration of the course of voriconazole is unclear: should therapy be continued until the fungal infection has cleared or neutrophil count has recovered, or should there be a cut-off period? Should therapy be limited to high-risk, immunocompromised patients with possibly life-threatening infections? Is the trial data for candidiasis and as empirical therapy convincing enough? Is it suitable to use the paediatric trial as a basis for treating children with fungal infections? The use of voriconazole may be limited due to the visual and hepatic side effects as well the potential for many drug interactions and mirtazapine.
