Stored at 70jC underwent three freeze-thaw cycles. Stock solution stability studies were performed at room temperature for 4 h and in refrigeration for 22 days. In addition, a long-term 28 days ; stability study was done in human plasma stored at 70jC. The stability of D7-MVA was assessed in an autoinjector for 20 h. Stock solution stability studies were performed at room temperature for 4 h and in refrigeration for 112 days. Stock solution stability studies were carried out at the MQC level, and working IS was prepared from fresh and refrigerated stock solutions. The drug and IS response ratios of stored and fresh stocks were compared. In other stability studies, five replicates of LQC and HQC were analyzed. Drug interactions: in repeat- and single-dose studies, there was no evidence of significant drug interaction on systemic exposure to fluticasone propionate and salmeterol when given alone or in combination via the diskus.

There were 1, 084, 100 women in the study. At recruitment the average age was 56 years. Follow up was 2.6 years of for breast cancer diagnosis and 4.1 years for breast cancer mortality. There were 9, 364 diagnoses of invasive breast cancer and 637 deaths from breast cancer. Most women were postmenopausal, and the main analysis was on 830, 000 postmenopausal women. Table 1 shows the main results, in terms of the number of diagnoses of breast cancer or breast cancer deaths per 1, 000 women, the relative risk, and the number of users of HRT needed to generate an extra case of breast cancer diagnosis or breast cancer death. Current use of HRT, but not past use, was associated with increased numbers of cases and deaths. Past use, even for less than a year, was associated with no increased risk. Over 2.6 years there was one extra diagnosis for every 260 women currently using HRT. Over 4.1 years there was one extra breast cancer death for every 16, 000 women currently using HRT.

To give the lowest dose of inhaled steroid compatible with asthma control. There is no indication at the present time for routine investigation of, or prophylactic treatment for, osteoporosis in patients on low dose inhaled corticosteroids. In patients receiving high dose therapy 1000 g beclomethasone or budesonide day or 500 g fluticasone day ; general measures to counteract osteoporosis such as regular exercise, hormone replacement therapy, smoking cessation, adequate dietary calcium ; should be considered. More long term follow up data are needed for patients on long term high dose inhaled steroids. Inhaled fluticasone propionate has become available in the UK since the last revision of the BTS guidelines. Its clinical eYcacy and safety have been extensively documented. Fluticaslne is as eVective as beclomethasone dipropionate and budesonide at half the dose when given by equivalent delivery systems.1316 At equipotent doses fluticasone may have the potential for producing similar systemic eVects to those of beclomethasone and budesonide. Flugicasone should be included in the guidelines as an alternative inhaled steroid at half the doses recommended for beclomethasone and budesonide when given by metered dose inhaler MDI ; . In addition to diVerences in oral bioavailability, all currently available inhaled steroids are also absorbed systemically from the lungs11 12 and there is increasing evidence that the delivery system is an important determinant of the systemic eVect of inhaled steroids. The detection of systemic eVects depends on the sensitivity of the indices used to detect them. Systemic eVects may be reduced by the use of a large volume spacer with an MDI or with mouth washing without swallowing, or with a dry powder inhaler. The Turbohaler delivers approximately twice as much inhaled steroid to the lung17 and doses should probably be halved when this device is used but, as in all cases, dosage should be titrated against control of asthma and treatment reduced when control is achieved. Although local and systemic side eVects show a relationship with increasing dose, it has often proved diYcult to demonstrate clear dose responses to the therapeutic anti-asthma ; eVects of inhaled steroids in controlled clinical trials. However, in clinical practice improved asthma control can often be achieved by increasing the dose. Current guidelines recommend that patients should double the dose of inhaled steroids temporarily if their asthma deteriorates or at the first sign of an upper respiratory tract infection. There are no published trials which have specifically addressed this question, but the recommendation should be retained though controlled studies are needed. There are no published controlled trials of the eVectiveness of nebulised budesonide in adults. See also section on "Gaining control and the stepwise approach to management" below and advil.

T hough the precise mechanism of analgesic action is unknown, a number of studies suggest that it interferes with the actions of those pharmacologically active substances that are initially released when injury occurs. It is helpful to parents when they are notified that their child is having asthma symptoms at school so they can adjust the controller medication taken at home. Controller medications prevent or reduce the bronchial inflammation. These medications are likely to be taken at home on a regular basis. Depending on the level of asthma control, these medications may increase or decrease throughout the school year. Controller medications: Are used to reduce inflammation and mucus in the airways. That are inhaled are not the same as anabolic steroids that are in the sporting news. Are prescribed for use on a regular basis, thus are generally used before and after school hours. Are not used for the treatment of sudden onset of breathing difficulty. Need to be taken for days or even weeks before they work to their full effectiveness Side Effects of inhaled controllers are: Hoarse voice, sore throat and a mild throat infection called thrush. Examples of inhaled controllers: Beclomethasone also sold as Vanceril or Qvar ; , Budesonide Pulmicourt ; , Fluticasons Flovent ; Some children may be taking a tablet to control their asthma. Examples of controllers in tablet form: Montelukast sodium Singular ; , zafirlukast Accolade and theophylline. Flohale rotacap fluticasone , flixotide , flovent ; used to prevent wheezing, shortness of breath, and troubled breathing caused by severe asthma and other lung diseases.

For montelukast, 85.8% for fluticasone, and 82.7% for fluticasone plus salmeterol ; also suggest that most patients fared well. An interesting finding was that there were relatively few upper respiratory infections in the montelukast group. This may have been due to a misdiagnosis of allergic rhinitis, for which montelukast is effective, or possibly to a more specific effect on leukotriene production due to viral infection.33 Alternatively, the chronic use of inhaled corticosteroids may reduce immunity, thereby predisposing patients to infection. In a recent trial involving patients with mild persistent asthma, there were minimal differences in asthma control between those receiving an inhaled corticosteroid twice daily only as needed, guided by a symptom-based action plan, and those receiving either a leukotriene modifier twice daily or an inhaled corticosteroid twice daily.34 The results of our study suggest another strategy, which is less complex for patients to follow. For patients whose asthma is well controlled with the twicedaily use of an inhaled corticosteroid, the use of once-daily fluticasone plus salmeterol or oncedaily montelukast could be considered. If treatment failure occurs, treatment could be modified accordingly. Whether the better outcomes found in our fluticasonesalmeterol group offset the convenience of a once-daily oral formulation montelukast ; depends on the preferences of patients and physicians as well as on cost. Recent studies have raised concerns about the potential adverse effects of long-acting beta-agonists and inhaled corticosteroids.35-37 Individual patients and their physicians must choose a treatment regimen for asthma that balances efficacy with actual or perceived risks and. Since 1983, the number of new antibacterial agents approved by the FDA has steadily declined. Moreover, according to a 2004 article in Clinical Infectious Diseases, researchers found only six new antibacterial agents disclosed in development programs at 15 pharmaceutical and 7 major biotech companies. Brad Spellberg, an author on the paper, said he "repeated the analysis in 2005 and there were no meaningful differences." Methodological comments G Allocation to treatment groups: states random, but method not described. G Blinding: states double-blind. Study medications packaged in identical 30-g tubes, each patient received four tubes. Twicedaily group and vehicle group received two tubes for morning and two tubes for evening containing either fluticasone or vehicle, respectively. Once-daily group received two morning tubes vehicle ; and two evening tubes fluticasone ; . No description of contents. G Comparability of treatment groups: no statistically significant differences between treatment groups with respect to gender, ethnic origin, age or baseline disease characteristics. Severity of signs and symptoms were comparable. No statistically significant difference between groups in percentage of patients missing at least one study medication, sites affected or sites treated. However, a greater proportion of patients in the twice-daily group had their arms evaluated. G Method of data analysis: all statistical tests were two-sided and at the 5% significance level. Comparison of the three treatments were made at baseline and at each post-baseline evaluation. With the exception of the mean change decrease ; in the total severity score, the p-values for the group comparisons once-daily or the twice-daily treatment vs vehicle and once-daily vs twice-daily treatment ; were based on the Van Elteren rank sum test, adjusted for investigator differences. For the mean change in total severity score, pairwise tests were made using a t-test. G Sample size power calculation: based on an expected difference between active and vehicle treatment groups of at least 25%. Given this assumption, 60 patients per treatment group were found to be sufficient to detect this difference with power of 80%. G Attrition drop-out: of 238 enrolled patients, 2 from each group did not return for any follow-up visits. 55 232 24% ; withdrew from study prior to completion of day 29 evaluation see table above ; . General comments G Generalisability: patients with an established diagnosis of eczema moderate to severe ; . G Outcome measures: not shown to be valid or reproducible. Subjective and rely on memory of condition at baseline, therefore possibility of recall bias. G Inter-centre variability: not reported. G Conflict of interests: none stated, but all data referenced to Glaxo Dermatology, Division of Glaxo Wellcome Inc. G Other: diagnosis of patients described as `eczema' rather than `atopic eczema'. Therefore, the reviewers sought clinical advice, which suggested that in view of exclusion criteria see above ; , these patients would likely have atopic eczema.

Low for prophylactic administration.62, 79 Prophylactic methemoglobin formation could present its own problems, however, for victims of smoke inhalation or in other settings where high levels of carbon monoxide occur.33, 80 A number of 8-aminoquinoline analogs of primaquine have also been studied as potential prophylactic drugs because they create elevated methemoglobin levels of long duration. Only preliminary information is available regarding the methemoglobin-forming potential of these drugs in humans.62, 81, 82. Mum level at approx. 4 h after the onset of stimulation [27]. To determine the effects of GCs on this response, HUVEC were treated with steroids for 5 min before the addition of TNF, and E-selectin levels were investigated by fluorescence-activated cell sorting analysis at 4 h. shown in Figure 1, TNF at 10 ng\ml strongly induced E-selectin expression and this response was inhibited by 5060 % ; , but not completely prevented, by GCs. For inhibition by dexamethasone and fluticasone, the approximate IC values were 10 and 0.1 nM respectively. Time course &! experiments revealed that the steroids were most effective when included before TNF and that the ability to cause inhibition of the cytokine effect was lost completely if steroid addition was delayed beyond the 12 h post-induction period results not shown and panadol. Attempt to back-titrate the inhaled corticosteroid dose to the minimum effective dose during periods of asthma control Drug utilisation data suggest that high doses of ICS are being used: the highest strength formulations of ICS are by far the most frequently prescribed12, 13 the average daily dose of flutocasone used is about 75% that of either beclomethasone or budesonide12 rather than the 50% that would be expected from the relative equivalent doses ; --similar experiences have been seen in the results of asthma case studies completed by GPs in New Zealand in 1999 2000. There is a risk that high doses of ICS are continued once a patient achieves asthma control. However, medium doses of ICS are as effective as high doses in maintaining asthma control.14 After 612 weeks of symptom control, attempt back-titrating the dose of ICS by about 25% every three months.1, 5 Particular attention is advised in the initial 23 weeks following a reduction in dose when relapse is most likely to occur. Is much more effective in reaching complete repigmentation than are FP and UV-A used alone, but large interindividual differences occur. Fluticasone propionate, UV-A, and a combination of FP and UV-A seem to be safe for long-term treatment of vitiligo. Arch Dermatol. 1999; 135: 1061-1066 Topical corticosteroids can be effective in the treatment of vitiligo.7, 8 It is wellknown that use of corticosteroids has an immunosuppressive effect. If vitiligo has an autoimmune pathogenesis, this could explain why the disease process stops when applying corticosteroids, thereby allowing melanocyte repopulation spontaneously or by UV irradiation. Repopulation of the epidermis with functional melanocytes in treated areas starts around hair follicles and appears as perifollicular islands of pigment that gradually coalesce. Also, a gradual spread of pigment from the margins of vitiligo lesions to the center takes place. Repigmentation has been shown to continue for 2 to 4 months after cessation of treatment. Repigmentation depends on the potency of the corticosteroid used.7 Use of a moderately potent corticosteroid often has. DPIS DELIVERING THE SAME DRUGS.--To accurately compare drug delivery by DPIs, one should compare devices containing the same drugs. Because the lung deposition of the Diskhaler is similar to that of the Diskus, fluticasone propionate is equally effective in children, whether administered by Diskhaler or Diskus.50 No effect on HPA-axis function was observed. Similarly, fluticasone plus salmeterol Advair ; delivered by the combination Diskus inhaler is as effective as the 2 drugs given in separate Diskus inhalers, because combining the 2 drugs in 1 inhaler does not change the delivery characteristics of either drug.51 Two crossover cumulative dosing trials compared salbutamol albuterol ; delivered by various DPIs.52, 53 The first compared the Diskhaler, Rotahaler, and Turbuhaler, with starting doses of 200 g for the Diskhaler and Rotahaler and 50 g for the Turbuhaler. Doses were then doubled for 4 doses.52 In the first study, salbutamol delivered by the Turbuhaler was twice as potent as that delivered by the Diskhaler and Rotahaler and advil!

Trade marks All of GlaxoSmithKline's pharmaceutical products are protected by registered trade marks in major markets. In general, the same mark is used for a product in each market around the world, but there may be local variations. For example in the USA the trade mark Paxil is used instead of Seroxat and Advair is used instead of Seretide. Trade mark protection may generally be extended for as a long as the trade mark is used by renewing it when necessary. GlaxoSmithKline's trade marks on pharmaceutical products generally assume an increasing importance when the patent for that product has expired in a particular country and generic versions of the product become available. In the Consumer Healthcare business trade marks are particularly important, as the business is very brand orientated and many products do not have patent protection. GlaxoSmithKline is routinely engaged in legal disputes in defence of intellectual property rights on many of its products see Note 30 to the Financial statements, `Legal Proceedings'.
