Herpes zoster and PHN are medical conditions not considered among these prevalent disease states because of the difficulty in identifying patients from the claims data. The diagnosis of these conditions is not captured at the pharmacy level and may not be coded as a primary diagnosis in the medical claims system. Thus, the true prevalence of these conditions and their economic burden in managed care plans are not widely known. The current medications used to treat herpes zoster infection also are indicated for multiple medical conditions, and the ability to track utilization specific to patients with herpes zoster or PHN is very difficult. The lack of integrated medical and pharmacy records prevents the routine evaluation of this patient population in our organization, however, like other managed care organizations MCOs ; , we are familiar with the comorbidities associated with herpes zoster and PHN. The challenge for MCOs is applying limited resources to manage multiple diseases and therapeutic categories of pharmaceuticals including the treatment of herpes zoster and PHN. MCOs have a responsibility to provide reasonable and effective formulary choices to physicians treating patients. In that respect, our organization, like the general managed care community, would be interested in the results of credible studies and the potential clinical and economic impact on medical and pharmacy management of diseases.
